Ironically, April is both Sexually Transmitted Disease (STD) Awareness month as well as Oral, Head and Neck Cancer Awareness month. Many people don't even realize how these two diseases are linked and how they both pose a serious health risk, especially for our adolescent boys.
As far as STDs, according to the United States Centers for Disease Control and Prevention (CDC), even though only a quarter of the sexually active population is made up of people age 15 to 24, this group comprises almost half of the newly diagnosed 19 million STD cases each year.
There's nothing we want more than to be healthy and thin. We want to throw on a pair of jeans and a t-shirt without wondering if it covers our fanny. We want to pull out last year's clothes without worrying about whether or not they're going to fit. And P-UH-LEASE! We want less clothes in our drawers -- but all one size!
So why is this goal so impossible to achieve? We have all the tools to succeed at our fingertips. Free weight loss advice on the Internet. Encouragement (and stern warnings) from our physicians and light cooking shows on TV. Most of us have even tasted victory on an occasion or two ... or four ... or more. With all the statistics and all the desire -- why is it so hard to rid ourselves of the burden that continues to squelch our happiness and our health?
Asian pop star Edison Chen, who fled Hong Kong in disgrace last year after nude photos of himself with several female celebrities were posted on the Internet, testify in an unusual court hearing started Monday in Vancouver. Chen's trouble began after he took his broken laptop in for repairs in Hong Kong. Shortly afterwards, explicit photos of him with different women, some identifiable as famous actresses, singers and models, began appearing online. After months of silence, Chen, a singer and actor who grew up in Vancouver, Canada and is one of Asia's most recognized celebrities, was to testify before a Hong Kong judge and lawyers over five days. The testimony had been expected to last five days, but both the prosecution and defense completed their questions Monday. Chen return Tuesday to read and sign the court transcript.
A Canadian judge and Hong Kong's chief magistrate will be co-commissioners at the hearing as Hong Kong police question Chen. Chen was ordered by the judge to answer a question raised by the defendant's lawyer to identify the four women in the string of sex photos. The four names are: Cecilia Pak-Chi Cheung, Gillian Yan-Tung Chung, Cecilia Pak-Wun Chan and Rachel Sze-Wing Ngan. His private sex pictures with several Hong Kong starlets set off an Internet firestorm but now he says he hopes the women involved will soon put this behind them. Edison Chen made the comment as he emerged from a British Columbia Supreme Court.
The hearing is taking place in Vancouver because Edison Chen, 28, fearing for his safety refused to return to Hong Kong to give evidence at the trial of computer technician Sze Ho-Chun, who is facing charges of distributing via the Internet pictures of the actor having sex with female celebrities. On the second day of the hearing, Chen added that 40 photos were all taken by ONE woman who had an affair with him in 2003. Guess who? The married one with the cuckold husband who had to get his baby DNA tested!

Chen's international fame skyrocketed after the photos appeared and Google said last December his name was the most searched term in Asia in 2008. Edison Chen's name was searched for more in the Chinese speaking world last year than the Beijing Olympics, the devastating earthquake in China's Sichuan province and the tainted milk scandal. And it's not like we haven't 'done' it before. Most of us have felt the excitement of a new beginning. We're going to do it this time! Day by day we plan and cook, look up and check off. But life isn't a series of peaks; there are valleys in the mix as well. Family struggles and work issues, relationships and holidays all demand a piece of us. Our focus shifts ever so slightly and we're once again struggling. It's not as easy as it was and we wonder why. The losses slow from pounds to ounces. Evenings seem to be getting looooonger. Then it happens -- the inevitable binge. Maybe it's a binge and maybe it's a binge!. But either way, 'perfection' is broken and we feel discouraged and afraid. Why is it so hard now when it was so easy before?
Actually, that's an excellent question and there's naturally not one answer. However, with all the time I've spent in weight loss meetings, I've seen one common theme. That is ...focus.
In the beginning of any new weight loss journey, no matter who we are and no matter what diet we're following, the diet is the focus of our time and attention. We go to the meetings. We read the material. We plan our days. We set aside the time to do it right. As time goes on and we're more comfortable with our relatively new way of living, our peripheral vision becomes broader. Life's clamor is always at the outer edges of our vision, ready to take center stage.
But our previous unhealthy way of eating is still so much a part of us that it's easy to slip back to the familiar. When emotions are strong and we are busy or tired, if we're not focused on our relatively new way of eating, it's no wonder we feel the relentless tug of our old friend, food.
Being comfortable with a new healthy lifestyle is definitely a goal we all are striving for. We just need to remember, that with the comfort level we can't let slip the focus that got us to that comfort level in the first place.
A recent study looked at sexual activity in the high school population. Published in theArchives of Pediatrics and Adolescent Medicine, this study found that for high school students, oral sex was most commonly their first sexual experience when they were beginning experimentation with sexual activity. It was twice as likely to precede vaginal intercourse than the other way around. Teens who engaged in oral sex by 9th grade were more likely to eventually have had vaginal intercourse by the end of 11th grade. In fact, often experimentation with oral sex led to riskier sexual activity within six months. The reverse was also found to be true: teens that delayed having oral sex were less likely to engage in vaginal intercourse during high school.
While many teens are aware of the dangers of STDs such as chlamydia and gonorrhea, HPV is another story. Some think it only concerns girls. More recently there has been a push to educate and vaccinate girls to prevent cervical cancer, but not much is being said about the potential problem it poses for adolescent boys.
HPV, according to the CDC, is a very common sexually transmitted disease infecting about 6 million people a year. It's estimated that 50 percent of sexually active men and women have been exposed at some point in their lives. In the majority of infections, our body's immune system takes care of it without any treatment. However, the same way certain strains of the virus get into cells of the cervix and change them into cancerous cells, it can also happen with the cells in the mouth and throat when exposed during oral sex.

Which brings us to the Oral, Head and Neck Cancer Awareness portion of April. According to the National Cancer Institute, around 65 percent of the cases of oropharynx cancer at the base of the tongue and tonsils are associated with HPV. Almost 80 percent of these cases occur in men. Recent research finds a rise in the incidence of head and neck cancers in this new demographic of younger men who never smoked or drank alcohol excessively. This younger demographic of patients were all found to be HPV-positive secondary to exposure during oral sex. This is corroborated by a CDC survey that found about 90 percent of males in general report having oral sex with partners of the opposite sex. Other studies have found that having six or more oral sex partners in a lifetime is associated with a three-times higher risk of HPV-induced oropharyngeal cancer.
A surgical colleague of mine agrees. He does research into the immuno-biology of head and neck cancers transformed by viruses such as HPV. Dr. Duane Sewell, M.D., associate professor of Otorhinolaryngology-Head and Neck Surgery at the University of Maryland School of Medicine, says "Despite the slight overall decrease we are seeing in the number of head and neck cancer patients associated with smoking and alcohol use, we are seeing a rise in a subset of younger men contracting these cancers and they are attributed to oral sex and HPV."
The good news is that these HPV-related head and neck cancers are somewhat easier to treat. Some studies have found better survival rates; almost 85 percent of nonsmokerswith HPV tumors survive compared to only 45 to 50 percent of those people who are HPV-negative and smoke. And another bright spot is that while the new vaccines for HPV have been found to prevent only about 70 percent of cervical cancers in women, they are much more effective at preventing head and neck cancers. Dr. Sewell states, "About 95 percent of HPV tumors are attributed to strain 16 which can be prevented by both commercially available vaccines."
Right now, the CDC only recommends vaccinating adolescent girls -- they came in just short of a recommendation for boys, stating physicians and parents have the option to vaccinate their sons if they choose. The public health community does advocate the vaccination of boys as a form of "herd immunity." That means that if we vaccinate boys, we will see fewer men walking around with HPV and, correspondingly, fewer women, as well. But in light of the recent study I discussed above, which found oral sex is the gateway to sexual activity for teens, as both the mother of an adolescent boy and as a physician, I think people should seriously consider the vaccine for their sons.
Another important caveat to learn from all this is that we need to speak frankly and explicitly with our teens about sex. We cannot be squeamish and dance around the words, even if it means using slang terms and specifics to get the point across. As uncomfortable as your kids or you may feel, don't let it stand in the way of their health. It's not worth it. Unlike cancer, no one ever really died of embarrassment!

A GRAPHIC video clip of a former Singaporean air stewardess and part-time actress having sex with a man has been circulating online.
The four-minute clip, which focused nearly entirely on the woman in explicit poses performing sex acts, is the latest in a growing and dangerous trend among young adults who take compromising pictures of themselves or let others do so.
But unlike the other clips that have surfaced online of relatively unknown local victims, the woman in this video is a celebrity of sorts.
The New Paper believes she is a part-time actress who has appeared in television dramas on the local Malay TV channel Suria.
The video looks to be taken by the man.
It also appears to have been taken with the actress’ knowledge, as she is the focus of the video throughout.
The woman in her 20s, a former air stewardess, did not want to say much about the video when contacted by The New Paper yesterday.
When asked if she was the person in the video, she simply said it was “personal info” she did not wish to divulge.
When asked if she was upset by the video, she replied: “If you were in the video, would you not be upset?”
A hotline caller who claimed she was a friend of the woman in the video said that the clip surfaced in the last two days on a US-based media-sharing site that also included two still photographs of the woman in a bikini posing with friends.
The site allows users to create, share and discuss blogs, photos, videos and music with others.
The video of the woman was taken down by yesterday evening.
According to support staff from the site, the account of the woman was created on Tuesday and cancelled by the creator earlier yesterday.
The support staff have also said that “if this was an illegal act and the authorities contact us with a subpoena, we will provide whatever information we can”.
This same clip of the woman and two others that showed her naked also appeared on a site popular with Malay artistes in the region.
Those too were removed within a day.
The friend of the woman said that the woman was “very upset” when bikini pictures of her appeared last year, but these videos were “shocking” in comparison.
The woman acted in several drama series.
The New Paper understands that the woman is no longer on contract with MediaCorp.
Those familiar with the woman’s TV appearances were “shocked and disgusted” by the video.
Mr Muhammad, a driver in his 30s, said that he found the link to the woman’s video through a Facebook friend’s profile and watched the video yesterday morning.
“I was shocked to see it, especially since she has not been on television for the last few years. It is strange that this has suddenly surfaced,” he said.
A media executive who only wanted to be known as Serena, 31, said: “The video was disgusting. Who knew that a ‘girl-next-door’ type like her could be so naughty.”
Lawyers and counsellors interviewed yesterday touched on the growing trend of “camwhoring” and the dangers it poses.
More people are open to the idea of taking such pictures these days, while the Internet and the wide range of gadgets with cameras make it that much easier to film, and distribute the images.
According to entertainment lawyer Samuel Seow from Samuel Seow Law Corporation, it is an offence to make such videos.
Said Mr Seow: “They ought to know that such content is considered obscene. As long as you are the one filming it, you are the maker of the obscene content.”
According to Section 29 of the Films Act, an offender can be fined between $20,000 and $40,000, or jailed for up to two years, or both, if found “making” or “reproducing” obscene films.
It is also an offence to possess obscene video clips or pornographic material. Those caught can be fined $1,000 per film, up to a maximum of $40,000.
In the case of a woman who is a willing party in the clip, Mr Seow said she “she cannot claim that someone has infringed her privacy”.
However, because the reputation of the woman is reduced through the dissemination of this private video, it can constitute “defamation” of the woman and she can try to file a defamation claim against the person who leaked it.
The Straits Times reported in July last year that counsellors say there is an increase in people who indulge in “camwhoring”.
Mr Mani Joseph, the assistant director of the Asian Women's Welfare Association Family Service Centre, said he sees about five young people who have taken such photos – some of whom have posted them online – each day.
This was rare in the past, he said. “Youth used to be shy. Now, they are bolder,” said Mr Joseph, who has counselled some who post topless photos of themselves online.
Dr Carol Balhetchet, director of youth services at the Singapore Children’s Society, said she has had to counsel one teen every fortnight against such behaviour. Such acts were previously unheard of, she added.
“Many don’t realise how easily the material can leak out to the Internet.”
